Confirm Insurance Coverage
We have created an Insurance Coverage Guide to help you determine your coverage when you call the Member Services number on the back of your insurance card. It is your responsibility to confirm coverage before scheduling an appointment.
Billing
We are an in-network provider for Blue Cross Blue Shield MN and United Healthcare and can bill direct. If we submit a claim for nutrition counseling and your claim is denied you will be required to pay the full insurance rate. Insurance is billed $300 for 90-minute initial consultation and $135 for 45-minute follow-up appointment. It is your responsibility to determine coverage prior to your appointment.
For reimbursement from out-of-network insurance providers we can provide you with a Superbill that you can submit to your insurance for possible compensation. Private pay appointments can be paid for with HSA or FSA funds regardless of insurance carrier.

BCBS or UHC Initial Nutrition Consultation
90-minutes | First Appointment Only
Fill out a comprehensive health history and other intake forms online prior to your appointment.
During this appointment Amber will review your history and goals and get to know you better.
You will leave with a clear goal and action plan to start your nutrition journey.
BCBS or UHC Follow-Up Appointment
45-minutes
Follow-up appointments are scheduled based on your personal nutrition plan and need for structure and accountability but the average is every 2-4 weeks. Number allowed may vary by plan.
4-6 months of regular appointments is the minimum suggested time to work together.
